In demonstrating that a group of calcium ion channels play a crucial role in triggering inflammatory responses, researchers at Beth Israel Deaconess Medical Center (BIDMC) have not only solved a longstanding molecular mystery regarding the onset of asthma and allergy symptoms, but have also provided a fundamental discovery regarding the functioning of mast cells.

Their findings appear in the January 2008 issue of Nature Immunology.

A group of immune cells found in tissues throughout the body, mast cells were once exclusively known for their role in allergic reactions, according to the study's lead author Monika Vig, PhD, an investigator in the Department of Pathology at BIDMC and Instructor of Medicine at Harvard Medical School. "Mast cells store inflammatory cytokines and compounds [including histamine and heparin] in sacs called granules," she explains. "When the mast cells encounter an allergen - pollen, for example - they 'degranuate,' releasing their contents and triggering allergic reactions."

But, she adds, in recent years, scientists have uncovered numerous other roles for mast cells, suggesting they are key to a number of biological processes and are involved in diseases ranging from multiple sclerosis and rheumatoid arthritis to cancer and atherosclerosis.

In order for mast cells to function, they require a biological signal - specifically, calcium. Calcium moves in and out of the cells by way of ion channels known as CRAC (calcium-release-activated calcium) currents. Last year, several research groups, including Vig's, identified CRACM1 as being the exact gene that was encoding for this calcium channel.

"With the identification of this long-elusive gene, we were able to create a knockout mouse that lacked CRACM1, and [as predicted] these animals proved to be resistant to various stimuli that usually cause severe allergic reactions," she explains. Further experiments demonstrated that mast cells removed from the CRACM1 knockouts were not able to take in calcium, and therefore, were unable to provoke allergic responses when they were exposed to allergens.

"These findings provide the genetic demonstration that CRAC channels are essential in mast-cell activation," notes senior author Jean-Pierre Kinet, MD, BIDMC Professor of Pathology at Harvard Medical School. "This provides the proof of concept that an inhibitor of the CRAC channel should be able to impact mast-cell related diseases, including asthma and allergic diseases."

Adds Vig, "Since mast cells are also known to contribute to the progression of several other debilitating diseases, including multiple sclerosis, rheumatoid arthritis and cancer, an inhibitor of the CRAC channel could, in the future, help in slowing the progression of these diseases as well as alleviate disease symptoms."

IONIA - While childhood obesity heads the list of problems for today's youth, and some experts call obesity an "epidemic," one question remains.

What's being done to stop it?

Because students spend the majority of their weekdays in schools, school officials are taking a hard look at the problem.

"There are a lot of things we're doing (at Ionia Public Schools) to deal with obesity," said IPS food service director Elenda May, who is also a chairperson on the Wellness Committee. "It's something we've been doing for awhile, actually. We're trying to impact students with the 'balance your life' mentality."

A good way for students to balance their lives is to first balance their meals.

Elementary schools across the district have removed pop machines from their buildings this year and have instead promoted juices or water.

In addition, IPS cafeterias across the district now offer healthier options inside the cafeteria.

Instead of highly processed breads, IPS and many other districts now use multi-grain buns.

In place of standard potato chips, which are extremely high in saturated fat and sodium, schools now offer baked chips instead, while french fries were moved from a daily option to only available once per week.

"So far, we've only had one person complain about [these changes]," May said. "We've definitely tried to focus on diets."

May has insisted on low-fat pizzas and chicken nuggets, and now each cafeteria in the district offers a low-fat grilled chicken meal each day as a healthy option.

They've also gone to serving fresh fruit and vegetables in the high school cafeteria at discounted prices to encourage students to eat healthy.

These are all big steps, said May, because improving food intake while at school - in the cafeteria and out - goes a long way toward improving student lifestyles.

"What we want to do is change the way students think about food and exercise at a young age, so that as they get older, those habits will be ingrained in them," May said. "Otherwise, they keep those bad eating habits."

Physically, the message from the school - and community - is clear.

"We've worked to encourage involvement between schools and the community to get children up and moving," said MSU Extension Educator Brenda Long, who has written grants that have brought in $16,000 to Ionia through the Michigan Department of Health in the past two years.

That money was used to promote nutrition and physical activity education through the Ionia Youth Workgroup.

"In order to beat something as prevalent as obesity, we need to see education coming from a lot of different places. The school is teaching health in the curriculum, it's one place. But it needs to come from the family and community," she said.

Different schools across the district have held recess/after-school programs to spur youth activity.

Jefferson Elementary principal Jack Manciu has implemented run-walk programs after school in the past, as has Rather Elementary principal Darin Magley.

Similarly, various youth fitness clubs can be found across the county as a direct result of rising obesity rates.

May said some schools in the district actually push their staffs to model proper physical and nutrition behavior through the 'Six Weeks to Wellness" program.

Staff members get points for certain things like walking a mile, eating healthy foods and the like.

"The important thing is that students are seeing the example," May said. "It's an ongoing process."

The city has gotten involved, too.

The Ionia Parks and Recreation Department offers a myriad of youth sports at free to low-cost fees.

Youth open basketball, indoor/outdoor soccer, Tee Ball and Tae-Kwon-Do are just some of the youth options available for children of almost any age.

They also offer exercise classes for adults and older children that range from low intensity to high intensity.

There's also open basketball each Tuesday and Thursday at the Armory gym for anyone old enough to compete.

"We have a lot of things for kids to do," said Parks and Recreation Director Matt Painter, who hopes more parents get involved. "Even our river trails are an option. Anything that gets people out and burns some calories helps keep obesity in check."

Extreme Obesity Affects Chances Of Kidney Transplantation

For patients on the waiting list for a kidney transplant, severe and morbid obesity are associated with a lower chance of receiving an organ, reports a study in the February Journal of the American Society of Nephrology.

Led by Dr. Dorry L. Segev of the Johns Hopkins University School of Medicine, the researchers used data from the United Network for Organ Sharing (UNOS) to examine how obesity affects waiting times for kidney transplantation.

"The results identify a potential bias in organ allocation that is not consistent with the goals of our allocation system," Dr. Segev comments. "Obese patients are waiting longer for kidney transplants when compared with their non-obese counterparts, even after adjusting for all medical factors tracked through UNOS."

Dr. Segev and colleagues analyzed data on more than 132,000 patients wait-listed for kidney transplants from 1995 to 2004. As obesity increased, the likelihood of receiving a transplant decreased.

Adjusted for other factors, the chances of receiving a kidney transplant were 27 percent lower for patients classified as severely obese and 44 percent lower for morbidly obese patients, compared to normal-weight patients. (The chances of receiving a transplant were not significantly reduced for patients classified as overweight or mildly obese.)

In addition, when a kidney became available, patients in the highest categories of obesity were more likely to be "bypassed"--that is, their physician was more likely to decline the offer of a kidney. The chances of being bypassed were 11 percent higher for severely obese patients and 22 percent higher for morbidly obese patients.

The findings raise concerns that obesity may be a previously unappreciated source of bias in organ allocation. "It is possible that providers are bypassing obese patients and instead transplanting non-obese patients because they feel that kidneys are a scarce resource and they want the kidneys to go to the patients who will benefit most from them," says Dr. Segev. "However, there is strong evidence that even obese patients will benefit significantly from a kidney transplant. And more importantly, the U.S. organ allocation system is not based on such medical decisions, but instead is based on the notion that everyone who gets listed deserves a fair chance at getting transplanted."

The bias might play out in other ways as well. Obese patients tend to have worse outcomes, which may reflect badly on the results achieved by doctors or hospitals--possibly even affecting quality measurements, which are increasingly tied to reimbursement. In addition, the reimbursement for transplant surgery is the same for difficult or complicated cases as for simpler cases. "These two practices generate a strong disincentive against challenging cases, and could potentially contribute to the bias against obese patients that we saw in our study," says Dr. Segev.

Dr. Segev notes that matching an available organ with an appropriate recipient requires clinical judgment, which could not be fully captured in this study. "However," he adds, "it is unlikely that these factors could explain such dramatic observations."

This study is scheduled for publication in the February issue of the Journal of the American Society of Nephrology (JASN) and early online publication.

When hypnosis is used to make people forget, it produces measurable changes in the brain that suggest the effects are real and not simply people "letting themselves go".
About two thirds of the population can be hypnotized and common uses include the treatment of pain, anxiety and phobias, Telegraph.co.uk said.
However, sceptics have argued that hypnosis does not result in an altered state of consciousness--a 'trance'--but is an exaggerated form of social compliance, where subjects suspend their critical faculties to do whatever a hypnotist asks of them.
Now brain scans of people that are taken following a hypnotic suggestion to forget have revealed parts of the brain really are affected.
The researchers team that did the study say their insights into memory suppression and recall may help understand the mechanisms underlying some forms of amnesia, along with how people suppress distressing memories or things they would rather not dwell upon.
Prof Yadin Dudai and colleagues at The Weizmann Institute of Science, Rehovot, describe in the journal Neuron a study on two groups of volunteers--those who were susceptible to hypnotic suggestions and those who were not--after they had shown a documentary depicting a day in the life of a young woman.
A week later, they placed them in a brain scanner and induced them into a hypnotic state. In this state, the scientists gave the subjects a posthypnotic suggestion to forget the movie, also giving them a reversibility cue that would restore the memory.
Once the subjects had been brought out of the hypnotic state, the researchers tested their recall, then gave them the reversibility cue and tested their recall again. As expected, the hypnosis-susceptible group showed reduced recall of the movie, compared with the hypnosis-non-susceptible group.
Analysis of the brain scans revealed distinctive differences between the hypnosis-susceptible group and-non-susceptible group in specific brain areas--occipital, temporal, and prefrontal areas.
"The surprise for us was that activity was raised during memory suppression in one specific region in the frontal cortex." In effect, it probably tells the other brain regions "don't even think about retrieving that memory", he says.
"The one thing we can say for sure is that hypnotism worked under the conditions we used," says Prof Dudai, adding that the findings are different from those seen in people who attempt to deceive. "We are therefore highly confident that this is not an artifact," he noted.

"Obesity is a natural extension of an advancing economy. As you become a First World economy and you get all these labour-saving devices and low-cost, easily accessible foods, people are going to eat more and exercise less," health economist Eric Finkelstein said.
In The Fattening of America, published this month, Finkelstein says adult obesity more than doubled in the US between 1960 and 2004, rising from 13 per cent to around 33 per cent. "With our ever-advancing modern medicine there helping to save the day (at least for many people), are government and the media blowing the magnitude of the 'obesity crisis' out of proportion?" his book says.
A study in which Finkelstein and colleagues at the RTI International, an independent research institute in North Carolina that works on social and scientific problems, asked overweight, obese and normal weight people to predict their life expectancy came up with a total difference of four years.
Normal weight respondents predicted they would live to 78, the obese to 74, and the overweight 75.5.
Other studies that looked at death data back the conclusion that people who carry excess weight tend to die slightly earlier, the book says, and draws the conclusion that "many individuals are making a conscious decision to engage in a lifestyle that is obesity-promoting".
"People make choices, and some people will choose a weight that the public health community might be unhappy about. Why should we try to make them thinner?" Finkelstein said. Linda Gotthelf, a doctor who heads research at Health Management Resources, a private, nationwide firm that specialises in weight loss and management, agreed that Americans now live longer but stressed that quality of life declines with age. "People are living longer but with more chronic diseases," Dr Gotthelf said.
"That brings a diminished quality of life, especially for the obese who have more functional limitations as they age and tend to be on multiple medications." Dr Gotthelf disagreed that people chose to be obese. "There are studies in which people have said they would rather lose a limb or be blind than obese. Being obese is not a desire," she said. "For many, this is a problem they have struggled with for many years... it gets discouraging after a while," she said. "I would not doubt that if you asked obese people if they could push a button and not be obese, close to 100 per cent would say they would push the button." Finkelstein says he wrote The Fattening of America to "encourage discussion of what I understand is probably an uncomfortable position for a lot of people."
Even if private industry and government take steps to protect society against the costs of obesity, many Americans "will likely continue to choose a diet and exercise regimen that leads to excess weight," because losing weight requires too many lifestyle sacrifices, his book warns.

In theory and practice, traditional Chinese medicine is completely different from Western medicine, both in terms of considering how the human body works and how illness occurs and should be treated. As a part of a continuing system that has been in use for thousands of years, it is still employed to treat over one-quarter of the world's population. Since the earliest Chinese physicians were also philosophers, their ways of viewing the world and human beings' role in it affected their medicine. In TCM, both philosophically and medically, moderation in all things is advocated, as is living in harmony with nature and striving for balance in all things. Prevention is also a key goal of Chinese medicine, and much emphasis is placed on educating the patient to live responsibly. The

Chinese physician also is more of an advisor than an authority; he or she believes in treating every patient differently, based on the notion that one does not treat the disease or condition but rather the individual patient. Thus two people with the same complaint may be treated entirely differently, if their constitutions and life situations are dissimilar. Disease is also considered to be evidence of the failure of preventive health care and a falling out of balance or harmony.

There is some confusion in the West about the fundamental philosophical principles upon which traditional Chinese medicine is based—such as the concept of yin and yang, the notion of five elements (wood, fire, earth, metal and water), and the concept of chi—yet each can be explained in a way that is understandable to Westerners.

Yin and yang describe the interdependent relationship of opposing but complementary forces believed to be necessary for a healthy life. Basically, the goal is to maintain a balance of yin and yang in all things.

The five elements, or five-phase theory, is also grounded in the notion of harmony and balance. The concept of chi, which means something like "life force" or "energy," is perhaps most different from Western ideas. TCM asserts that chi is an invisible energy force that flows freely in a healthy person, but is weakened or blocked when a person is ill. Specifically, the illness is a result of the blockage, rather than the blockage being the result of the illness.

Besides these philosophical concepts that differ considerably from infection-based principles of medicine and health, the methods employed by traditional Chinese medicine are also quite different. If allopathic Western practitioners could be described as interventionist and dependent on synthetic pharmaceuticals, TCM methods are mostly natural and noninvasive. For example, where Western physicians might employ surgery and chemotherapy or radiation for a cancer patient, a TCM physician might use acupuncture and dietary changes. TCM believes in "curing the root" of a disease and not merely in treating its symptoms.

Another major difference is how the patient is regarded. In Western medicine, patients with similar complaints or diseases, usually will receive virtually the same treatment. In TCM however, the physician treats the patient and not the condition, believing that identical diseases can have entirely different causes. In terms of the principles upon which it is based and the methods used, traditional Chinese medicine, therefore, is considered by many in the West to be a radically different system of healthcare.

To some in the Western world, this very strangeness is the reason why it might be attractive. To others, tired of what they perceive as their physician's perfunctory, analytical, and sometimes cold manner, TCM offers a more humane, patient-oriented approach that encourages a high degree of practitioner-patient interaction and is not overly dependent on technology.

For example, during a consultation with a TCM practitioner, the patient will receive a considerable amount of time and attention. During the important first visit, the practitioner will conduct four types of examinations, all extremely observational and all quite different from what patients usually experience.

First, the practitioner will ask many questions, going beyond the typical patient history to inquire about such particulars as eating and bowel habits or sleep patterns. Next, the physician looks at the patient, observing his or her complexion and eyes, while also examining the tongue very closely, believing that it is a barometer of the body's health and that different areas of the tongue can reflect the functioning of different body organs. After observing, they listen to the patient's voice or cough and then smell his or her breath, body odor, urine, and even bowel movements. Finally, the practitioner touches the patient, palpating his or her abdomen and feeling the wrist to take up to six different pulses. It is through these different pulses that the well-trained practitioner can diagnose any problem with the flow of the all-important chi. Altogether, this essentially observational examination will lead the physician to diagnose or decide the patient's problem. This diagnosis is very different from one in contemporary Western medicine. No blood or urine samples are tested in a laboratory. The key to this technique lies in the experience and skill of the practitioner.

After making a diagnosis, the physician will suggest a course of treatment from one or all of the available TCM methods. These fall into four main categories: herbal medicine, acupuncture, dietary therapy, and massage and exercise. A typical TCM prescription consists of a complex variety of many different herbal and mineral ingredients. Chinese herbal remedies are intended to assist the body's own systems so that eventually the patient can stop taking them and never becomes dependent on them. Herbal formulas are usually given as teas, which differ according to the patient.

Other common techniques used in a TCM prescription are as follows:

Acupuncture is based on the notion that the body's vital energy force, chi, travels through known channels or "meridians." The acupuncturist inserts tiny, thin sterile needles at particular, selected points on the body to unblock or correct the flow of energy. These needles are hardly felt as they are inserted and are left in place for 15–20 minutes. Some patients report immediate improvement, others feel exhilarated, while some feel like sleeping. In some cases, patients say their condition worsens before it improves. No contemporary scientific explanation exists as to how or why acupuncture works.
Moxibustion is a variation sometimes employed. Moxibustion is the slow burning on or over the body of special herbal "cones." These are placed on specific acupoints and provide penetrating, relaxing heat.
Massage is often recommended, and a deep finger pressure technique known as acupressure is often used to promote the proper flow of chi.
Diet is considered essential to good health, and what might be called "kitchen medicine" is just another aspect of herbalism. One example is a delicious dong quai black bean soup that is traditionally eaten by women in China after childbirth and each menstrual cycle.
Therapeutic exercises are sometimes prescribed as well. In both the exact and flowing movements of t'ai chi, and the breathing techniques of Qi Dong exercise is considered essential to relieving stress and promoting the smooth flow of chi.

As a system of total healthcare, TCM is prepared to deal with any physical or mental problem, condition, or disease. However, unlike Western medicine at its best, TCM is not able to render the kind of emergency crisis intervention that saves lives during physical traumas. Nonetheless, it works best at achieving its goal of practicing preventive medicine. It has proven effective in treating many types of aches and pains and in helping people with depression and fatigue, as well as circulation and digestive problems. Overall, its emphasis on good diet and exercise, as well as on individual responsibility and moderation in all things, suggest that it is grounded in fundamentally sound principles.

Have you ever been asked to take photos for a wedding? For some, wedding photography is a casual affair. Take a point-and-shoot camera, then snap away when the opportunity arises. For others, it's a serious, full-fledged business.

Wedding photos are a challenge to take, simply because of the flurry of activities that go on and the wide variety of lighting conditions that may occur. This article will serve as a guide for those starting out in wedding photography, and will delve into various tips and tricks.

Preparation

One of the most critical elements in being a successful wedding photographer is preparation. You absolutely need to prepare ahead (at least 2 months' ahead) of time. Get to know the bride and the groom, understand their needs and wants. Some good questions to ask are:

How many photographers will there be?
What type of photos would they like? Mostly formal group portraits? Or mostly candid shots of the couple?
How many guests will there be at the wedding? Are they mostly family members or friends?
Which locations will be used at the wedding? A church? Will there be a reception or a dinner?

Asking pertinent questions like these will help you understand the requirements for the wedding photos and thereby take better shots.

Locations, Events and Times

Once you've understood what the wedding locations, events and times are, find some time to go scout around those places.

For a church wedding, I usually like to go inside the church to check out the lighting, décor and environment. I try to find the correct places to position myself so I don't mess up during the wedding day. Do take note that some churches do not allow flash photography during the wedding ceremony. So you may need to get a fast lens.

It's also important to check out places like the reception area, or the podium where the speech will be made. If there will be a wedding dance, then check out the dance floor and the seating arrangement if possible.

Organize Your Gear

This is a critical step. I usually prepare 1 month ahead of the wedding date just to be sure I have time to get any equipment I've missed out. Here's a list of critical equipment to bring for the wedding day shoot:

Digital camera: This one is obvious. It's good to have a backup analog camera as well, just in case.
Camera Lenses and filters: Take along any special lenses or filters you want to use.
Tripod: Another critical piece of equipment, important for indoor shots under low light.
Memory cards: Bring at least two 1 GB cards. I'm not kidding – you won't believe how many shots you'll take during a wedding.
Portable drive: This is critical in case you run out of space on those memory cards.
Assistant: Try to bring along an assistant. It could be a friend or a help at the wedding. He or she can help you arrange the group before you take photos, taking a lot of stress off you.

On the Wedding Day

Ok, let's assume the actual wedding day, has arrived. Here's a typical sequence of events to help guide you on the wedding photography process.

At the House

If you're taking shots of the bride getting ready, please be early. In fact, you should be one of the earliest to arrive so that you can set up your positions, tripods or whatever equipment you need to use.

At the Church

Once you're at the church, find out where the priest will be standing. Position yourself in an appropriate position. Some critical locations where wedding photos must be taken include the entrance to church, the aisle and the place where the couple will stand.

And here's another tip – it's important not to get too excited about your shots so early on the wedding day. Remember, you have a long day for taking photos. I once did a wedding where I snapped so many photos at the church entrance, I nearly ran out of memory card space by the time the couple exchanged vows!

If there happens to be a church reception, make sure you're out to the reception area faster than the rest of the guests, and perch yourself in the correct position. Receptions are a good time to capture candid and fun shots, so knock yourself out.

Formal Group Shots

If there are any formal group shots to be taken, be sure your assistant is there to help you. Try to start with the big groups, then slowly remove family members so that you can get the various combinations of family and friends required.

Cleaning up

What do you do after shooting the wedding photos? Well, plenty! You need to do some basic image editing, like removing red eye, cropping and rotating. You also need to burn the photos into CDs and DVDs, (usually in a slideshow format). If you're in the wedding photography business, you should know that you cannot delay this step. Try to get the CD or DVD final package out to the happy couple as soon as you can.
